Overview:
Male – Age 41, Social worker. Fertility Analysis (as of May 2004): low concentration – 10.7 million, very low normal forms (morphology) – 1%, very low after wash total motile count – 6.1 million, small testes. Recommended ICSI (intracytoplasmic sperm injection).

Initial Intake – History
Story: He and his wife had been trying for 2.5 years to get pregnant. They have done IVF/ICSI in the past with good response but no successful pregnancies. They are now preparing for another cycle of IVF/ICSI.
