High E2?

I am a 38 y.o woman struggling with infertility for the second time.  I have a 2 year old conceived via IUI and superovulation after being diagnosed with unexplained infertility.  All of my fertility testing came back "normal" at that time and I was told that I had an excellent response to meds.  A little went a long way.  We are now trying to conceive again, but seem to be infertile again this time.  My CD3 bloodwork done by my GP 10 days ago has me worried.  The E2 level was 274, progestorone 3, FSH 6.4, LH 7.7.  DHEA extremely low.  I have well controlled mild lupus and frequent migraines, and take low dose prednisone, plaquenil, inderal, zomig PRN.  I am awating an appointment with a fertility specialist.  What can you tell me about the possible causes of the high E2?  How quickly do I need to act on this?

E2 tested in Canada or the United States

Are your labs from Canada or the United States?  They are measured differently.  If in the states then yes it is elevated.  If in Canada they are within normal range.

 

I really like the OAR test for measuring ovarian reserve which includes the hormones you had tested plus AMH and Inhibin B.  http://www.acubalance.ca/accurate-assessment-ovulatory-egg-supply-available

hi Dr. Brown.  Thanks for your response.  My E2 was tested in Canada.  Specifically the estradiol was 274 pmol/L on cd3.  Ref range 110-184 pmol/L for mid-follicular phase.  Progesterone was 3 nmol/L.  Ref range <2 nmol/L for mid-follicular phase.  FSH was 6.4 IU and LH was 7.7 IU.  Estradiol and progesterone seem to be above the ref range, so this is my concen.I'd love to hear that those values are normal! I must admit that I am a little confused by the Canadian measurements as I had fertility testing and treatment last time in Boston.  Please let me know what you think of these most recent cd 3 tests run in Canada.  My worst fear is that unbeknowst to me my biological clock will run out while awaiting treatment.  Thanks for the info and link regarding ovarian reserve assessment.  I am waiting to be seen at Genesis in Oct some time, so perhaps they will suggest this testing.
